Client Forms
This page provides all the necessary online forms for Furmiliar Comfort services. To ensure a smooth and efficient process, please complete and submit the forms online before our consultation. Be sure to fill out all fields with as much detail as possible. You may write “none” or “N/A” where applicable. After submission, you will receive an email with a PDF copy of your completed forms for your records.

1. Client Information (for pet sitting clients)
This form is for pet sitting clients and contains your basic information, emergency contact, travel itinerary, house sitting requests, information about your home, and home access arrangements.
2. Pet Information (for pet sitting clients)
Please fill out one form per pet. If you have a group of animals such as a flock of chickens or a colony of rabbits, then one form may be used per group, insuring to indicate how many are in the group.
3. Dog Walking & Excursions
This form is required for dog walking clients only.
4. Veterinary Release
This form is required for all services except house sitting without any pets or animals.
5. Service Agreement
This form is required for all services.
6. Guest WiFi Access
Primarily for live-in visits and in-home daycare visits, this form provides information on allowing guest WiFi access.
